Earlobe Repair - Restoring the Original Piercing in One Stage

For many patients, earlobe repair is not just about restoring appearance. It is about getting back to something they love - in this case, wearing earrings.

One-stage earlobe repair - ZNG Plastic Surgery, Singapore

A patient came to me with a near-complete earlobe cleft. Over many years of wearing earrings, the piercing had gradually stretched until it could no longer hold the earring in place and she reported bouts of inflammation previously. She was not bothered by the appearance of the earlobe so much as the loss of function. She just wanted to wear her earrings again.

Standard techniques would repair the cleft directly and require her to wait at least two to three months before re-piercing. But when I asked whether she would consider piercing at a different site, her answer was clear: she wanted the same hole, in the same place.

Lobuloplasty - ear lobe repair by Dr Ng Zhi Yang, Singapore Plastic Surgeon

That changed the repair goal. The closure had to not only restore the earlobe but recreate a functional piercing site at the same spot - precise in position, stable enough to be used again, and designed to avoid the linear scar contracture that would otherwise develop over time. Fundamentally, this is a test of execution of Plastic Surgery principles.

The procedure was performed under local anaesthesia as a single visit. Sutures were removed at seven days, and at that appointment she tested the repair with her own earrings. It worked.

She was so glad, in her own words, that she did not have to wait two to three months.

A Note on Earlobe Repair

Stretched or torn earlobes are more common than many people realise, particularly in patients who have worn earrings for decades. The gradual downward pull of heavy or pendulous earrings slowly elongates the piercing until it splits or can no longer support jewellery.

Repair is a straightforward procedure under local anaesthesia, but the technique needs to match the patient's goal. For patients who want to reuse the same piercing, and particularly those who want to do so as soon as possible, the repair approach must account for this from the outset, not as an afterthought.
